The Form Ext is generally used for development purposes and requires knowledge of the physical file system behind the site.
On our vanilla Site Borchester, Ext should be in a standard location so that choosing "Page" should work, but the templates can be adjusted to choose alternative locations if you so desire. However doing so requires knowledge of the physical file structure of the site.
